The DRK and German Youth Red Cross demand mandatory first aid training in school curricula with 24 hours per school year, including lay resuscitation and AED training. North Rhine-Westphalia is already implementing resuscitation courses in schools with specialized teacher training. This systematization addresses dramatically declining lay resuscitation rates (only 38.4% with phone guidance in 2025) and could significantly improve survival chances in cardiac arrest long-term.

The Bundestag discussed emergency care reform in July 2026: Emergency dispatch centers (112) and physician on-call services (116117) are to be digitally networked, telemedicine and mobile response teams established. Emergency departments are overwhelmed nationwide (exit blocking at 85-90% occupancy), reform aims to bring relief through better triage and advance notification. Organizations such as DRK, Johanniter, and Malteser demand reliable funding and genuine participation in implementation.

Medical evidence shows: CPR + AED doubles survival chances compared to CPR alone. Latest technology (QShock™) enables first defibrillation in under 8 seconds. Movement toward AED availability in public spaces and schools is intensifying; Bremen and other states are installing devices in parks and sports facilities. The gap between existing knowledge and implementation remains critical.

Wiesn 2026: Aicher Ambulanz reported 70 paramedics with specialized self-defense training – an indicator of rising violence against emergency personnel. Studies show: targeted Tactical First Aid training significantly improves survival chances in attacks/knife injuries. The trend toward security training for emergency responders is intensifying; emergency departments report increased assaults by patients and relatives.

Standard courses compressed from 16 to 9 teaching units (45 min./unit), content remains the same according to BAGEH, didactic modernization according to ERC guidelines. Mandatory for driver's licenses and workplace first aiders, training follows European Resuscitation Council standards. Compression aims to improve compliance and faster basic qualification without reducing critical competencies.
